12. Nerivio Access Plan - Terms & Consent

About the Plan

The Theranica Plan is a user- and time-limited offer providing supported access to Nerivio therapy designed to generate informative real-world evidence and user feedback. The Plan forms part of Theranica's commitment to Public and Patient Involvement and Engagement (PPIE). We are offering this Plan to help make our product more accessible and to better understand how it works for patients in real-world use. By participating, you'll also help us improve the product through your feedback.

The Plan enables eligible patients to access the therapy for a reduced price of £99. This price represents an estimated 60% discount on the standard full price of £249. The company may terminate the Plan at any time under its sole discretion.

Who Can Join

To be eligible, you must:

• Be based in the United Kingdom
• Have an existing Migraine diagnosis from a Healthcare professional, or successfully complete the pre-purchase screening questions
• Have access to the product
• Be willing to actively participate in the Plan

What Participation Means

As part of the Plan, you agree to:

• Use the product as instructed
• Regularly complete your treatment diary and feedback in the app
• Provide accurate and honest information about your experience

Participation requires ongoing engagement. If activity is incomplete, access to the Plan may be discontinued at our discretion.

Your Data & Privacy

• Your data is collected and processed in accordance with our App Terms & Conditions and Privacy Policy.
• All data is handled securely and in compliance with applicable data protection laws, including UK GDPR.
• Your feedback may be used, in anonymised or aggregated form, to improve the product and patient experience.

Safety

Important - Temporary Plan

• This Plan is provided as a temporary, limited-time offer.
• The Company reserves the right, at its sole discretion, to modify, suspend, or terminate the Plan, in whole or in part, at any time and without prior notice.
• Participation in the Plan does not create any entitlement to continued access, ongoing supply of the product, or future Plan benefits.
